Microsoft has made Forms available for personal use. Anyone with a Microsoft account can use the survey, poll, and test tool for free. An Office 365 subscription is required for extensive functionality.

Microsoft made Forms available to Office 365 Education customers in 2016 and Office 365 business subscribers in 2017, and added Forms for personal use on Thursday. Anyone with a Microsoft account can use Forms for free.

This is the full service, but paying Office 365 users have more options such as a larger amount of templates and the possibility to use a larger amount of respondents. Forms is a browser-based service and the Office Mobile app for Android and iOS that allows users to create and share questionnaire forms.

The tool provides templates for a variety of applications such as quizzes, party invitations and feedback questionnaires. Users can share the forms with a link or qr code and see the results directly in Excel for further analysis. The service competes with Google Forms, among others.
